The health intake questions you answer during registration serve two purposes: they confirm the test is appropriate for you, and they give the licensed physician the information needed to authorize your test order. The intake questions are short, specific to your test, and required — without them, the lab cannot process your sample.
How your answers are used
Eligibility screening. Your answers confirm the test is a good fit for your situation — for example, that you meet any age requirements and that there is no health reason the test should not be used.
Physician authorization. In the United States, lab testing requires a licensed healthcare provider to order the test. A licensed physician reviews your intake answers and, when appropriate, authorizes your test order. Without this review, the lab cannot process your sample. The physician assigned to your order is licensed in your state and reviews every order individually.
What if my answers raise a flag?
If your intake answers indicate the test is not appropriate for you, our support team will contact you by email or phone to explain the issue and work through next steps. You will not be left guessing about what happened or what to do next.
If you think you answered a question incorrectly — a typo in your date of birth is a common example — contact our support team through chat right away. A corrected answer can often resolve the issue quickly.
Is my health information protected?
Your intake answers are health information. They are handled under the Consent and Authorization for Services and the Privacy Policy you accept during registration. Your answers are used to process your order, support the physician's review, and deliver your results. They are not sold or shared beyond what is needed to fulfill your test.
