All three consents are required. Without them, a licensed physician cannot be assigned to your order and the lab cannot run your test.

Consent and Authorization for Services
The Consent and Authorization for Services authorizes the people involved in your test — the laboratory, the reviewing physician, and your test provider — to collect, use, and share your health information with each other so your test can be performed and your results delivered to you.
Every test order is reviewed and authorized by a licensed physician, and your results are reviewed by a physician as well. This document confirms you understand and agree to that clinical review. It is about making your test work, not about marketing your data.
Privacy Policy
The Privacy Policy explains how your personal and health information is handled throughout the testing process — what is collected, how it is used, how it is protected, and the choices available to you. Read it to understand how your data is kept private and secure.
Terms of Service
The Terms of Service is the service agreement. It covers how the testing service works, what it is designed for (wellness and health optimization, not medical diagnosis), what is expected of you as a member, and what you can expect from the service in return.
