To register your kit, you will need your Kit ID from the box and a few pieces of personal information you already know. Registration takes a few minutes. Having everything listed below ready before you start will let you move through the process without interruption.
Complete list of what you need
The following eight items are required to complete registration. You cannot submit your registration without all of them.
Kit ID — printed on the inside of the box and on your instruction card.
First and last name
Phone number
Email address
Street address — including apartment or unit number, city, state, and ZIP code.
Date of birth — used to confirm you are 18 or older and to correctly match your sample to you at the lab.
Health intake answers — a short set of questions specific to your test. A licensed physician reviews your answers to confirm the test is appropriate for you and to authorize your order before the lab can process your sample.
Three consents — you will read and accept the Consent and Authorization for Services, the Privacy Policy, and the Terms of Service. All three are required.
Why accuracy matters
The information you enter during registration is how the lab matches your sample to your identity and how the physician authorizes your test. Entering incorrect or incomplete information — including a wrong date of birth or a mistyped address — can delay your results or require you to re-register. Use your own accurate details every time.
Your contact details (phone and email) are also how we send shipping updates, lab notifications, and your results notification.
