How do I register my kit?

Step-by-step instructions for registering your kit before you collect your sample.

Registering your kit takes a few minutes: scan the QR code or visit the link on your instruction card, enter your Kit ID and personal details, answer a short set of health intake questions, and accept the consents. You must complete registration before you collect your sample — registration is what links your sample to you and allows a licensed physician to authorize your test.

What you need before you start

  • Kit ID — printed on the inside of the box or on your instruction card

  • Your personal details — name, phone number, email address, street address, and date of birth

  • A few minutes to answer short health intake questions and review three consent documents

How to register your kit

Follow these steps in order. Do not collect your sample until you have completed Step 6 and received your confirmation.

  1. Find your Kit ID. It is printed on the inside of the box or on your instruction card.

  2. Open the registration site. Scan the QR code on your instruction card with your phone's camera, or type the registration website address from the card into any browser. If the QR code will not scan, go to the registration site directly and enter your Kit ID manually.

  3. Enter your personal details. You will be asked for your first and last name, phone number, email address, street address (including apartment or unit number, city, state, and ZIP code), and date of birth.

  4. Answer the health intake questions. These are a short set of questions specific to your test. A licensed physician uses your answers to confirm the test is appropriate for you and to authorize the order.

  5. Review and accept the three consents. The three documents are the Consent and Authorization for Services, the Privacy Policy, and the Terms of Service. All three are required.

  6. Submit. After submitting, you will see a confirmation on screen that your kit is registered. Once you see that confirmation, you are ready to collect your sample.

If you do not see a confirmation after submitting: Do not collect your sample yet. Contact our support team through chat and we will confirm your registration status.

Common questions

Lost your Kit ID? Contact our support team with your tracking number, full name, and shipping address and we will look it up for you.

Using someone else's phone or computer? That is fine — registration works on any device. Just make sure you enter your own details, not the device owner's.

Why does registration have to happen before collection? The lab cannot process an unregistered sample. Registration is how the lab matches your sample to you and how the physician authorizes your test. Collecting before registering causes delays and in some cases may require a new sample.
